Pre-Admission Assessment
Avana provides an assessment of each resident's care needs prior to admission to begin developing individualized care plans.

Assistance with activities of daily living
Prompts, cues, and reminders to perform daily activities
Medication management and Pharmacy-delivery services
Assistance with safe transfers and ambulation
Toileting and incontinence care

On Call Physicians
For the convenience of our residents, we have partnered with a mobile physician service to provide on-site patient care and medication reconciliation services that are covered by most health insurance plans. Residents may choose to continue to see their own doctors as well.
